This page explains what Forma is, what it is not, and what you are agreeing to when you move forward. It takes about three minutes to read.
01 — What Forma Does
Forma is a technology platform. We collect your health information through a structured intake, match it to a protocol, and connect you with a licensed physician who reviews your case and decides whether to prescribe.
Forma does not practice medicine. Forma does not prescribe anything. Every prescription decision is made independently by the licensed physician who reviews your file.
02 — The Physician Decides, Not Forma
The physician who reviews your intake is an independent licensed medical doctor. Forma does not tell that physician what to prescribe, how much to prescribe, or whether to prescribe at all.
If the physician reviews your information and determines that a protocol is not appropriate for you, they will decline to prescribe. This is normal, it is legal, and it is the right outcome. Forma does not override that decision.
You are not guaranteed a prescription when you pay for a consultation. You are paying for an independent physician to review your case.
03 — These Medications Are Not FDA-Approved for Most Uses
The compounds in Forma's protocols are compounded by licensed 503A pharmacies under a patient-specific prescription. Compounded medications are not the same as FDA-approved brand-name drugs.
The FDA has not evaluated these specific compounds for safety and effectiveness in the way it reviews prescription drugs through the full approval process.
Compounded means the medication is prepared specifically for you by a licensed pharmacy. It is not a generic version of an approved drug.
There are no FDA-approved indications for most peptide uses. The prescribing physician exercises clinical judgment based on available evidence and your individual health history.
The evidence base for many peptide protocols consists of preclinical data and limited human studies. Results seen in animal research do not always translate to human outcomes.
04 — No Guaranteed Results
Nothing on Forma's website, quiz, or marketing constitutes a medical guarantee or a promise of specific outcomes.
Bodies respond differently to every intervention. Your baseline hormone levels, diet, sleep, training, genetics, and other medications all affect outcomes. Testimonials and case studies reflect individual experiences and are not typical results.
05 — This Is Not Emergency Care
Forma is not equipped to handle medical emergencies, acute illness, or urgent clinical situations.
If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
Forma's physician partners provide asynchronous or scheduled video consultations. They cannot respond in real time to acute medical events.
06 — Telehealth Has Limits
A telehealth consultation is not the same as an in-person exam. The physician cannot perform a physical examination, listen to your heart or lungs, or assess findings that require direct contact.
The physician may require laboratory work before prescribing. If labs are required, you will be directed to a lab partner or instructed to obtain labs locally.
The physician may determine that your case requires an in-person evaluation before a protocol is appropriate. In that case, they will not prescribe through the platform.
Certain health histories may make you ineligible. The intake form is designed to surface these, but it is not exhaustive. Be complete and honest in your responses.
07 — What the Compounding Pharmacy Is and Is Not
Your prescription, if issued, will be filled by a licensed 503A compounding pharmacy operating under state board of pharmacy oversight.
Compounding pharmacies are not FDA manufacturing facilities. Their products are not subject to the same pre-market approval, batch release testing, or post-market surveillance that FDA-approved drugs undergo. If you have a concern about a specific medication you receive, contact the pharmacy directly using the information on your prescription label.
08 — Privacy and Your Health Information
The information you provide during intake is protected health information under HIPAA. It is shared with the licensed physician for clinical review and with the pharmacy for prescription fulfillment.
Forma does not sell your health information.
09 — Supplement Recommendations
Forma may recommend dietary supplements alongside a peptide protocol. These supplements are not prescription medications and are not part of the physician's prescription.
Dietary supplements are regulated by the FDA as foods, not drugs. They are not required to prove safety or effectiveness before being sold. Forma earns a commission if you purchase a supplement through a link on this platform. This is disclosed on each page where affiliate links appear.
